Accelerating software complexity has created a multi-headed challenge for software developers. The criticality, complexity and volume of software have ballooned, creating an exponential increase in software risk. Company profits in automotive, aerospace and medical are crippled by software-defect product recalls while, according to Forrester, avionics and defence projects commonly experience schedule and budget overruns by a factor of two and deliver only 60% of the expected functionality.

 

Up to 85% of these errors stem from a failure to properly manage requirements. Boeing and Airbus maintain that the expense of identifying an error during first flight increases costs up to 900 times than if the error had been identified early in the development process.

AutomotiveCustomers demand more personalisation (e.g., seat, climate, and light controls), infotainment (e.g., trip management, weather reports, Internet), and advanced performance (hybrid propulsion or anti-lock braking), the number of lines of software code per automobile is expected to triple by 2011. General Motors VP Larry Burns has stated, “It’s pretty clear the auto industry is entering into a period of radical acceleration in the shift of the car’s DNA from being mechanically centered to being an electronically centered machine.”· Toyota recalls 160,000 cars with hybrid engines in 2005 due to a failure of its engine control software. · Nissan recalls 16,365 Murano and Infiniti EX 35 vehicles in February 2008 due to a software problem causing airbag failure.
· May 2008, Chrysler recalls about 25,000 Jeep Commanders to repair transmission control software that allows the engine to stall at high speeds and about 50,600 vehicles in February 2007 to reprogram antilock brake software.
Aerospace and DefenceModern aerospace designs rely on software- intensive systems to lower weight requirements and provide numerous mission-critical functionality, including power, start-up and navigation systems.· Delivering many combinations of mission- critical options requires additional software and longer delivery times for these “systems of systems.” These types of software-intensive projects tend to overrun their budget and schedule by a factor of 2 and deliver only 60% of the expected functionality. Medical DevicesSoftware-driven advances offer the opportunity to improve the quality of healthcare while reducing costs.  Software has already emerged in smart devices such as cardiovascular x-ray machines and in CT and MRI scanning processors that help increase the comfort of doctors and provide better diagnosis and results for complex surgery.· Defibtech issues a worldwide recall of two of its defibrillator products due to faulty self-test software that may clear a previously detected low battery condition. The issue affects approximately 42,000 units worldwide. (February 2007)
· Guidant Corporation issues a recall of certain defibrillators implanted in 21,000 patients due to a software memory error that may rarely limit available therapy. (June 2005)
